Optimised care for cardiovascular diseases through digitalisation

Cardiovascular diseases are the most common cause of death in Germany and require not only timely detection, but often also long-term treatment of chronic conditions. To support optimal care with digital solutions, the Federal Ministry of Education and Research (BMBF) has launched the Digital Progress Hub CAEHR (CArdiovascular diseases - Enhancing Healthcare through cross-sectoral Routine data integration). Three model regions are testing how the optimised, timely provision of relevant health information and the establishment of intelligent data-driven services can enable better healthcare for patients. In the process, health data from outpatient and inpatient care is standardised and structured and made usable for individual patient care in a research-compatible electronic health record.

An important aspect of this is the way in which patient data can be collected, processed and made available. SVA was already involved in the conception phase with medPower®, the software solution from the field of telemedical care that has been successful for years. In the further course of the CAEHR project, medPower® now provides a portal that offers cardiologists in private practice a comprehensive overview of patient data, in particular important information, survey responses from questionnaires and patient documents. The collected data is processed by means of intelligent data analyses and made available to physicians for therapeutic decision support. This creates an important interface between outpatient care and clinics.

Since medPower® can be used as a platform independently of the end device, "questionnaires" relevant for the care of cardiac patients can also be completed via an app. Depending on the requirements and infrastructure, the hybrid solution can be made available both on servers in clinics and in the cloud.
